Washington County Long Term Care Services

There are several Long Term Care Programs available to potential clients. They include Home and Community Based Services (services in the home) and placement in nursing and alternative care facilities. Eligibility is based on financial criteria and the client meeting the level-of-care (service eligibility) for these programs.

There are two applications that must be completed simultaneously. This is very important in establishing the first date of eligibility:

1. Financial eligibility determination This is the Medicaid application which you do through our County Departments of Social/Human Services

2. Uniform Long Term Care Form (ULTC 100) - for service eligibility. This form is then reviewed by a state designated peer review organization (PRO). The case manager will assist with decisions on the type of long term care program best suited to meet your needs.
